Good design improves the whole house, not just the new bit. The changes should balance the downstairs and upstairs, the receptions and bedrooms, so that the investment of money into the new build is reflected in the overall value.
Of course, the new build will have wow factor but adding, for example, a loft bedroom to a four bedroomed terraced house with an average sized kitchen and two small reception rooms won't make the house appeal to the sort of family looking at five bedroomed houses since the downstairs will still be too small.
Equally, building across the rear of a house, blocking up windows will create internal rooms made less valuable due to the lack of light. Yes, the additional floorspace will add value but this needs to be balanced against the impact on the rest of the house.
